Clover Meadows is a premier sub-acute rehabilitation center and long term care facility offering a comprehensive range of services and highest level of care and quality of services.

At Clover Meadows, our goal is to provide exceptional rehabilitation services and skilled nursing care in a nurturing, home-like living environment. Clover Meadows offers long-term care with skilled nursing, respite services and sub-acute care.

Whether the patient stays for a short term or plans to make Clover Meadows their new home, our staff will develop a program suited to the needs of each individual. Our supportive and caring staff is on call and available 24 hours a day to assist patients and their families with any questions, concerns or issues.

Sub Acute Care

Since the goal of Clover Meadows is to assist all patients in returning to the most independent living environment as quickly as possible, our dedicated staff helps patients reach their recovery goals successfully through an outcome-orientated approach to care.

 

Whether recovering from surgery, coping with chronic illness, or recovering from an injury, our dedicated team of doctors, therapists, nurses and other health care professionals will work together with the patient and his or her family to design a goal-oriented plan of care for each patient.  This plan will help the patient regain achieve the maximum outcome and return to the most independent living environment as quickly as possible. Clover Meadows’ sub-acute care division provides the best possible option for a quicker recovery and a return to independence. Physical therapy is offered 7 days a week, if indicated by a plan of care, to accelerate recovery times.

Clover Meadows offers long-term care and services  to meet the physical and emotional needs of individuals with chronic illnesses and disabilities or who can no longer care for themselves.

Whether you or a loved one are looking for a permanent arrangement or temporary living, Clover Meadows can offer a comfortable, professionally supervised environment with all the comforts and warmth of home. Clover Meadows is committed to creating an atmosphere of compassion, caring and excellent nursing care.

Respite

Sometimes a caregiver needs a brief respite from the day to day responsibilities of caring for a loved one, either to rejuvenate or to take a pre-planned vacation.

 

 

If you or your loved one needs a little additional temporary medical or nursing support, Clover Meadows provides short term respite care on a daily, weekly and/or monthly basis.  During a Respite stay, short-term patients receive the same high level of care as other patients, in a safe and secure environment cared for by skilled professionals. Clover Meadows Respite Services include:

Nurses like Heather, Janine, Natallia, Dori, Chi Chi and a host of others played a big part in my recovery. They endured my painful nights, comforted me, and assured me I would be alright. Other staff like Laeneese unit secretary kept my appt. made sure I was aware and made them. Nicki the laundry lady kept my clothes clean and Val, Housekeeping Director knows his house. PT was great I actually wanted to go. Recreation program is keeping residents busy, I even became a bingo addict. Overall Clover Meadows is still a facility that provides well-rounded care for the community and long-term care.As a resident of Clover Meadows for the last 30 days I have encountered excellent staff included dietary and housekeeping. The nursing staff are excellent and considerate. The physical therapist are great and compassionate.
